How Do I Qualify for The Medical Marijuana Program in Maryland?

  • Maryland’s “Written Certification” Process for Medical Marijuana

    Any person who lives in Maryland can participate in the program. A person from out-of-state who is in the state for the purpose of receiving medical care can be issued a written certification and obtain medical cannabis.
    There is no Maryland State Fee to be registered as a medical marijuana patient. You may request a Medical Marijuana Card from the State for a $50 fee.

    Get Pre-Qualified first by completing the medical records release online, allowing MDMMCC to request your records from your family physician. If you have a copy of your medical records you may fax or email them to us for a Free Pre-Qualification before your appointment.

    STEP 1: Before your appointment with MDMMCC you must first register online with the Maryland Medical Cannabis Commission. If you  are not computer literate our staff will be happy to help you register in our offices before you see our marijuana doctor.

    What Medical Conditions can MDMMCC Doctors issue a written Certification for Medical Cannabis?

    STEP 2: In order to be considered for a written certification for Maryland Medical Marijuana, you must have one of the following qualifying conditions, or you are receiving treatment for a chronic or debilitating disease or a medical condition that causes:

    • cachexia,
    • anorexia,
    • glaucoma
    • severe nausea
    • wasting syndrome,
    • severe or chronic pain,
    • post-traumatic stress disorder (PTSD)
    • seizures, or severe or persistent muscle spasms.
    • A chronic or debilitating disease or medical condition that results in being admitted into a hospice facility or receiving palliative care.

STEP 3: MDMMCC Doctors will perform an in-person, evaluation to determine if medical marijuana may benefit you. Once approved for your written certification, our MDMMCC doctor will register your certification into the Maryland MMCC registry system.

STEP 4: You will then be issued a Written Medical Cannabis Certification.  Additional evaluation and or follow up evaluation will be at the discretion of the MDMMCC physician.

STEP 5: Once you receive your written marijuana certification, you may then purchase medical marijuana from a licensed Maryland Medical Marijuana dispensary. (dispensaries will be open in mid 2016).

It’s quick and easy to obtain your medical records, and it will help you get your written certification sooner.

Contact Your Primary Care Doctor

Most practices require you to fill out a specific form to formally request your medical records. To obtain this form, simply call your primary care doctor’s office, or the office of the doctor who gave you the diagnosis, and request a copy of your records. They should be able to deliver it to you by fax, email, snail mail or you may pick it up from the doctor’s office.

NOTE: MDMMCC Doctors recommends that all patients should have a copy of their personal medical records, just in case you may need them in a hurry for an unforeseen emergency.

If your doctor’s office doesn’t have a specific form, simply write a letter to request your records. Be sure to include:
· Your name, including your maiden name (if applicable)
· Social security number
· Date of birth
· Address and phone number
· Email address
· Record(s) being requested
· Dates you were under the doctor’s care
· Signature
· Delivery option (pick up, fax, email, etc.)

What if I Don’t Have a Primary Care Doctor?

If you do not have a primary care doctor or have not had a check-up in a while, MDMMCC Medical Marijuana Certification Clinics will be glad to assist you by referring you to a doctor who will be your primary care physician and will evaluate your medical condition to determine if you have a qualifying condition.
